Students in grades 4-8, teachers, and parents across the state are now invited to participate in the 11th annual Illinois 5Essentials surveys, administered online by UChicago Impact at the University of Chicago. Participation in these surveys helps districts understand the condition of their schools and guides improvement. Reports will be sent to our schools and district in May 2023.
Your participation in the parent survey is much appreciated, as results are included in the annual Illinois School Report Card.

Students’ identities will be completely confidential, and teachers and administrators will never see individual responses to survey questions, only summary-level responses. As a parent, your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
Student Opt-out Form
Under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c) (1) (A), you have the right to review a copy of the questions asked of your student(s). Sample survey questions from last year are available on the 5Essentials website. If you DO NOT want your child to participate, please complete the opt-out form and return it to your school’s main office by February 20. Shabonee students will be surveyed on February 22, and Wood Oaks students will be surveyed on February 23 during the regular school day.
